What is an FUE hair transplant?
FUE — Follicular Unit Extraction — is the technique behind most modern hair transplants — you may also see it called FUE hair implants, FUE hair restoration or FUE hair replacement, but they all mean the same procedure. The surgeon removes follicular units, the natural groupings of one to four hairs, one at a time from the permanent donor area at the back and sides of the scalp, then implants them into thinning or bald areas where they keep growing for life.
The defining feature of FUE is what it does not leave behind: no strip, no stitches, no linear scar. Instead it leaves tiny circular marks less than a millimetre across, which heal and disappear among the surrounding hair — so you can wear your hair short without a tell-tale line across the back of your head. That single advantage is the main reason FUE overtook the older strip method to become the default choice for most patients worldwide.
This is a surgical procedure, carried out under local anaesthetic across a single day. It does not create new hair; it redistributes the DHT-resistant hair you already have, which is why careful planning and honest assessment matter as much as surgical skill.
FUE is one route within our full guide to hair transplants in Turkey — this page focuses on the FUE method itself.
How FUE works, step by step
- Design and planning. Before anything is touched, the surgeon assesses your donor density, maps the areas to be treated, and designs a hairline with you. This is the step that most determines whether the result looks natural. A hairline that suits a 35-year-old should still suit them at 55 — which means resisting the temptation to place it too low.
- Anaesthetic. The donor and recipient areas are numbed with local anaesthetic. The procedure itself is not painful once you are numbed; the only real discomfort is the initial injections.
- Extraction. Using a micro-punch, usually 0.7 to 1.0 mm in diameter, the surgeon removes follicular units individually. Punches can be manual or motorised; what matters more than the tool is the operator’s care in scoring at the correct angle and depth so each graft comes out intact.
- Graft preparation and storage. Harvested grafts are kept in a cooled preserving solution while extraction continues. The less time grafts spend outside the body, and the more gently they are handled, the better they survive — which is why unhurried, well-staffed surgery beats a rushed high-volume day.
- Channel creation. Tiny incisions are made in the recipient area at the precise angle, depth and direction of natural hair growth. Get this wrong and even perfect grafts grow in the wrong direction.
- Implantation. Grafts are placed one by one, single-hair units at the hairline for a soft, natural edge and denser multi-hair units behind them for coverage.
The whole process takes roughly six to eight hours depending on the number of grafts, and you go home — or back to your hotel — the same day.
Manual, motorised and robotic FUE
Clinics often advertise the extraction tool as if it were the whole procedure. It helps to understand the options — and why the hands using them matter more than the brand name.
- Manual FUE — the punch is turned by hand: slow and demanding, but it gives an experienced operator maximum control over angle and depth.
- Motorised (micromotor) FUE — a powered punch speeds up extraction and is the most common approach today. Gentle in careful hands; damaging if rushed.
- Robotic FUE (e.g. ARTAS) and devices such as NeoGraft — machines that assist harvesting. They can be consistent, but they are only as good as the team supervising them, and they do not design your hairline or place your grafts.
The honest takeaway: no tool guarantees a good result. Graft survival and a natural hairline come from careful, unhurried, physician-supervised technique — not from a machine on the brochure.
FUE, Sapphire FUE and DHI: the same family
These names cause a lot of confusion, so here is the plain version. All three extract follicles the same way — individually, the FUE way. The difference is in the details:
- Classic FUE opens recipient channels with steel blades, then places the grafts.
- Sapphire FUE uses sapphire blades for the channels, which can allow finer, denser work at the hairline.
- DHI implants each graft directly with a Choi pen, combining channel and placement in one step, often without shaving the recipient area.
None is universally “best”; the right choice depends on your hair, your goals and a specialist assessment.
Compare them: FUE vs DHI

FUE vs FUT: why we focus on FUE
The older method, FUT (follicular unit transplantation, or “strip”), removes a strip of scalp and dissects it into grafts. It can harvest a lot of grafts in one session and is sometimes cheaper, but it leaves a fine linear scar that limits how short you can wear your hair.
| FUE | FUT (strip) | |
|---|---|---|
| Donor scar | Tiny dots, no line | Fine linear scar |
| Wear hair short? | Yes | Limited by the scar |
| Donor healing | Faster, more comfortable | Slower |
| Who it suits | Most patients | Very large sessions on a budget |
| Offered at Clinic Elia | Yes | No (comparison only) |
For most international patients the absence of a linear scar makes FUE the clear choice. To compare FUE with DHI instead, see FUE vs DHI.
No-shave (unshaven) FUE
Standard FUE usually trims the donor (and often recipient) area short. No-shave — sometimes called unshaven or “long-hair” FUE — leaves your existing hair its normal length, so daily life is less disrupted while you recover.
It is genuinely useful for smaller cases and for people who cannot step away from work or public life. But it is slower, suits fewer grafts, and is not right for every case — a larger session almost always needs at least the donor trimmed. Whether it fits you is a question for your assessment, not a default upgrade to pay for.
What makes a good FUE result?
Three things separate a natural, lasting transplant from an obvious one — and none of them is the brand of machine a clinic advertises.
Hairline design. A natural hairline is slightly irregular, softens at the temples, and sits at an age-appropriate height. Straight, low, dense hairlines are the clearest sign of a transplant.
Angle and direction. Native hair grows at specific angles that change across the scalp. Recreating them is painstaking manual work, and it is where surgeon experience shows.
Donor management. Your donor area is a finite resource. Over-harvesting it for a bigger single session — a common shortcut at high-volume “mills” — thins the donor permanently and limits future procedures. A good surgeon takes what your donor can spare, not what fills today’s target.
Men and women. FUE restores male-pattern loss and, increasingly, female-pattern thinning — though women more often need medical assessment first, as the causes of hair loss are more varied.
Recovery after FUE
Recovery has two phases: the scalp heals over about two weeks, and the hair grows in over twelve to eighteen months.
Small crusts form around each graft and flake away by day ten to fourteen — never pick them. The donor area heals quickly, and most people return to work and travel within a week or two. Between roughly weeks three and eight, the transplanted hairs shed. This “shock loss” is normal and expected: the follicle survives beneath the skin and regrows. New growth begins at three to four months, thickens by six to nine months, and reaches its final density at twelve to eighteen months.
Full detail: Hair transplant recovery timeline
| When | What happens |
|---|---|
| Days 0–3 | Mild swelling and redness; sleep semi-upright; careful first washes. |
| Days 10–14 | Crusts flake away; donor comfortable; most people back to work. |
| Weeks 3–8 (around month 2) | Transplanted hairs shed (“shock loss”) — expected and temporary. |
| Months 3–4 | New growth begins. |
| Months 6–9 | Noticeable density. |
| Months 12–18 | Final result. |
Results and how long they last
Transplanted follicles keep their genetic resistance to balding, so FUE results are permanent. Graft survival is high in experienced hands, though no ethical clinic promises an exact figure — your result depends on your donor supply, degree of loss and healing. Your untransplanted native hair can still thin over time, which is why a surgeon plans for the future and sometimes recommends medication alongside surgery.
FUE in Turkey: cost and how it works
Turkey performs a large share of the world’s FUE procedures, and prices at reputable clinics run well below UK, European and North American levels — driven by lower operating costs, not lower standards at the good clinics. At Clinic Elia, your FUE is carried out at Acıbadem — an accredited private hospital in Istanbul — under physician supervision. Your price depends mainly on graft count, so an accurate figure comes after a photo assessment. A typical trip is around three days, and a genuine all-inclusive package covers the procedure, medications, hotel, VIP transfers, a coordinator and twelve months of follow-up.
The warning worth repeating: the cheapest quote is rarely the safest. A suspiciously low price usually means no surgeon involvement, an over-crowded schedule, or costs that appear after you arrive.

What changes the price: mainly your graft count (a ~2,000-graft hairline costs less than 4,000+), then technique (DHI or unshaven work takes longer), and what the package genuinely includes. A real all-inclusive price covers your hospital, hotel, transfers, translation and 12-month aftercare — see our all-inclusive package.

Frequently asked questions
Is FUE permanent?
Yes. Follicles from the donor area keep their resistance to balding, so transplanted hair is permanent. Non-transplanted hair can still thin over time.
Does FUE hurt?
The procedure is done under local anaesthetic and is generally comfortable. Mild soreness for a day or two afterwards is normal.
Will FUE leave a scar?
No linear scar — only tiny dot marks in the donor area that hide in surrounding hair, even worn short.
How many grafts will I need?
It depends on your degree of loss and donor supply, commonly 2,000 to 4,000 or more. Send clear photos for a personalised estimate.
When will I see the final result?
New growth from three to four months, real density by six to nine months, and the final result at twelve to eighteen months.
How is FUE different from DHI?
Both extract follicles the same way. FUE opens channels then places grafts; DHI implants directly with a Choi pen, often without shaving the recipient area.
How long do I stay in Turkey for FUE?
Usually around three days, with remote follow-up after you fly home.
Is FUE done with a robot or by hand?
Extraction can be manual, motorised or robot-assisted, but the result depends on the team’s skill and care, not the machine. Your hairline is always designed and your grafts placed by people.
Can I have FUE without shaving my head?
No-shave (unshaven) FUE is possible for smaller cases and leaves your hair its normal length; larger sessions usually need at least the donor trimmed. We will tell you honestly what your case needs.
Can women have FUE?
Yes. FUE treats female-pattern thinning too, though women more often need medical assessment first. See female hair transplant.
