DHI — Direct Hair Implantation — is a refinement of FUE in which each graft is placed with a Choi implanter pen, giving the surgeon precise control over angle, depth and density, and often allowing treatment without shaving the recipient area. This page explains how it works, who it suits, and — just as importantly — when a different technique is the better choice.
What is a DHI hair transplant?
DHI is an advanced form of follicular unit transplantation. As with any modern hair transplant, individual follicular units — the natural groupings of one to four hairs — are removed one by one from the permanent donor area at the back and sides of the scalp, then placed where hair has thinned. What makes DHI different is how those grafts are implanted.
In classic FUE, the surgeon first opens tiny recipient channels, then places the grafts into them in a separate step. In DHI, each graft is loaded into a Choi implanter pen — a fine hollow needle with a plunger — and the incision and placement happen in a single movement. There is no separate channel-creation stage.
That single-step approach gives the surgeon direct, graft-by-graft control over the angle, direction, depth and spacing of every hair. And because the recipient area does not need to be pre-cut, DHI can often be carried out without shaving the area being treated — a genuine advantage for anyone who cannot, or would rather not, shave their whole head.
Like all follicular unit work, DHI is a surgical procedure done under local anaesthetic across a single day. It redistributes the DHT-resistant hair you already have; it does not create new hair. Careful planning matters as much as the tool.
DHI is one technique within our full guide to hair transplants in Turkey.
How DHI works, step by step
- Assessment and hairline design. Before anything is touched, the surgeon evaluates your donor density and the area to be covered, and designs a hairline with you. This step — not the brand of pen — is what most determines whether the result looks natural. A hairline should suit your face now and still suit it in twenty years.
- Local anaesthetic. The donor and recipient areas are numbed. The procedure itself is not painful once you are numbed; the only real discomfort is the initial injections.
- Extraction. Follicular units are harvested individually from the donor area with a micro-punch — exactly as in FUE. This leaves no linear scar, only tiny dot marks that heal and hide among surrounding hair.
- Loading the Choi pen. Each graft is carefully loaded, tip-first, into an implanter pen. This is meticulous work, which is one reason DHI is usually slower than standard FUE and often needs a larger technical team.
- Direct implantation. The surgeon presses the pen into the recipient site and depresses the plunger, creating the incision and setting the graft at the chosen angle and depth in one motion. Working graft by graft, single-hair units are placed at the hairline for a soft edge and denser multi-hair units behind them for coverage.
- Aftercare briefing. You leave the same day with written instructions and, at a responsible clinic, direct access to your team for questions during healing.
A DHI session typically takes six to nine hours depending on the number of grafts.
DHI, FUE and Sapphire FUE: the same family
These names cause a lot of confusion, so here is the plain version. All three extract follicles the same way — individually, the FUE way. The difference is only in how the recipient area is prepared and how grafts are placed:
- Classic FUE opens recipient channels with steel blades, then places the grafts.
- Sapphire FUE opens those channels with sapphire blades, which can allow finer, denser work at the hairline.
- DHI combines channel-making and placement into one step using the Choi pen, often without shaving the recipient area.
You may also see robotic or motorised implanter systems advertised. They can assist the work, but — as with the Choi pen — the result comes from the team’s planning and placement, not the device.
None of these is universally “best”. A good surgeon recommends the approach that suits your hair, your goals and your donor supply — and is honest when two options would give much the same result. For a full breakdown, read FUE vs DHI.
DHI vs FUE at a glance
| DHI | FUE | |
|---|---|---|
| Implantation | Choi pen, one step | Channels opened, then placed |
| Recipient shaving | Often not needed | Usually needed |
| Density control | Very high, graft by graft | High |
| Who it suits | Dense, detailed hairline work | Larger areas, efficient coverage |
| Speed / cost | Slower, usually pricier | Faster, usually lower |
Neither wins outright — see the full FUE vs DHI comparison.
Not sure where you stand?
Send a few photos of your hairline and crown. A physician reviews them and tells you honestly what is realistic — including when the honest answer is that surgery is not the right step yet.
Why patients choose DHI
- Precise placement. Angle, direction and depth are controlled graft by graft, which helps when recreating a natural growth pattern.
- Dense packing where it counts. DHI can place grafts closely, useful for a strong hairline or a defined area.
- Often no recipient shaving. Helpful if you want to keep the procedure discreet or cannot shave for work or personal reasons.
- Short time out of the body. Grafts move from extraction to placement quickly, which supports survival when the team is well organised.
- No linear scar. As with all FUE-based methods, only tiny dot marks remain in the donor area.
These are real advantages — but they depend entirely on the surgeon’s skill and the clinic’s care, not on the pen itself. Suitability and results vary; only an in-person or photo assessment can confirm whether DHI is right for you.
Is DHI right for you?
DHI is often a strong choice if you:
- Want maximum density in a defined area, especially the hairline or front third of the scalp.
- Would prefer to avoid shaving the recipient area.
- Have a healthy donor area and hair loss that has largely stabilised.
- Have realistic expectations about what your donor supply can cover.
It is less likely to be the first choice when very large bald zones need covering. Because DHI is slower and more labour-intensive per graft, classic or Sapphire FUE can often cover extensive areas more efficiently in a single day. An honest surgeon will tell you when a different method would serve you better, rather than selling the technique with the highest price tag.
DHI recovery timeline
A typical pattern — everyone heals a little differently, and your clinic’s specific guidance always comes first.
| When | What to expect | |
|---|---|---|
| Days 1–3 | Mild swelling and redness; sleep semi-upright; first gentle wash as instructed by your clinic. | |
| Days 4–10 | Small crusts form around each graft and begin to flake away. Never pick them. | |
| Days 10–14 | Crusts have cleared; the donor area is well on its way to healing. Most people are back to normal routines. | |
| Weeks 3–8 (around month 2) | Weeks 3–8 | Transplanted hairs shed — this “shock loss” is normal and expected. The follicle survives beneath the skin. |
| Months 3–4 | New growth begins. | |
| Months 6–9 | Density builds and the result starts to look like your own hair. | |
| Months 12–18 | Final result: full thickness and maturity. |

Results and how long they last
Transplanted follicles keep their genetic resistance to balding, so a DHI result is permanent. Graft survival is high in experienced hands, though no ethical clinic promises an exact percentage — your outcome depends on your donor supply, your degree of loss and how you heal. Your untransplanted native hair can still thin over time, which is why a good surgeon plans for the future and may recommend medication alongside surgery to protect it.
How much does DHI cost in Turkey?
DHI usually costs a little more than standard FUE, because it is slower and more technique-intensive — but at reputable Turkish clinics it still sits well below UK, European and North American prices, driven by lower operating costs rather than lower standards.driven by lower operating costs rather than lower standards. At Clinic Elia, your DHI is carried out at Acıbadem — an accredited private hospital in Istanbul — under physician supervision. Your price depends mainly on the number of grafts, so an accurate figure only comes after a photo assessment.
A typical trip is around three days, and a genuine all-inclusive package covers the procedure, medications, hotel, VIP transfers, an English-speaking coordinator and twelve months of follow-up. As always, the cheapest quote is rarely the safest — read hair transplant cost in Turkey for the full picture.
Before you book: a safety checklist (for any clinic)
- Confirm a qualified clinician is accountable for your procedure — not technicians alone.
- Ask who performs each step, and how many procedures the clinic runs per day. Unhurried surgery beats high-volume “mills”.
- Expect an honest graft estimate based on your photos, and a plan that protects your donor area for the future.
- Get the all-inclusive price in writing, with no vague extras that appear after you arrive.
- Be wary of guarantees of an exact survival percentage or an unusually low, hairline-lowering promise.
Turkey leads the world in hair transplants precisely because its best clinics are excellent — but standards vary, so choosing carefully matters. See is Turkey safe for a hair transplant?

Frequently asked questions
Is DHI better than FUE?
Neither is universally better. DHI offers precise, dense placement and often no recipient shaving; FUE can cover large areas efficiently. The right choice depends on your hair and goals, and a good surgeon will be honest about which suits you.
Does DHI hurt?
The procedure is done under local anaesthetic and is generally comfortable. Mild soreness for a day or two afterwards is normal.
Can DHI really be done without shaving?
Often, yes — DHI can be performed as an “unshaven” or partially shaved procedure for the recipient area, which is why many patients choose it. Whether it suits your case depends on the area being treated; your clinician will advise.
Will DHI leave a scar?
No linear scar — only tiny dot marks in the donor area that hide among surrounding hair, even worn short.
How many grafts will I need?
It depends on your degree of loss and donor supply. Send clear photos for a personalised estimate rather than relying on a generic number.
When will I see the final result?
New growth begins at three to four months, real density develops by six to nine months, and the final result matures at twelve to eighteen months.
How long do I stay in Turkey for DHI?
Usually around three days, with remote follow-up after you fly home.
Is DHI done with a robot?
Extraction and implantation can be manual, motorised or robot-assisted, but the result depends on the team’s skill, not the machine. Your hairline is designed and your grafts placed by people.
Does the donor hair grow back after DHI?
The individual follicles removed do not grow back, but they are taken sparingly across the donor area so it stays evenly covered — which is exactly why over-harvesting must be avoided.
When can I exercise after DHI?
Light activity after a few days; avoid heavy exercise, sweating and swimming for about two to four weeks, following your clinic’s advice, to protect the healing grafts.
Are the before-and-after photos online real?
Not always — it is the most-faked thing in this field. See what a genuine result looks like in our honest before & after guide.
