Exactly what to expect after your procedure — day by day and month by month — including the shedding phase that surprises people, honest aftercare rules, and when your final result arrives.
Recovery has two parts
Recovery from a hair transplant (FUE or DHI) happens in two stages:
- Wound healing — the first ~2 weeks, when the scalp heals and crusts clear.
- Hair growth — a much longer phase over 12–18 months, when transplanted follicles shed, rest and then grow in.
The most important thing to understand early: your hair will look worse before it looks better (the shedding phase is normal). Patience and good aftercare are what protect your result. At Clinic Elia your recovery is supported under physician supervision at Acıbadem, with a direct line to your team in Istanbul. Everyone heals a little differently — always follow your own clinician’s instructions.
The full timeline
| When | What happens |
|---|---|
| Day 0 | Procedure day; crusts and redness; back to your hotel the same day. |
| Days 1–3 | Swelling settles; sleep semi-upright; first gentle wash. |
| Days 4–7 | Crusts form around grafts; redness begins to fade. |
| Days 7–14 | Crusts flake away; donor healed; most return to work/travel. |
| Weeks 3–8 (around month 2) | Shock loss — transplanted hairs shed; the follicle survives. |
| Months 3–4 | New growth begins. |
| Months 6–9 | Density develops and becomes clearly visible. |
| Months 12–18 | Final, permanent result. |
Day 0 — Procedure day
Done under local anaesthetic. Tiny crusts and redness appear in both donor and recipient areas. You go back to your hotel the same day with written aftercare and medications.
Days 1–3 — Early healing
- Some swelling (forehead/around the eyes) is normal and settles within days.
- Sleep semi-upright (~45°) to reduce swelling.
- First gentle wash as instructed by your team — usually around day 2–3.
- Do not touch, scratch or rub the grafts.
Days 4–7 — Crusting
- Small scabs (crusts) form around each graft.
- Continue gentle daily washing exactly as advised to soften and clear crusts.
- Redness begins to fade.
Days 7–14 — Crusts clear
- Crusts flake away gradually — never pick them (picking can dislodge a graft).
- The donor area is largely healed.
- Many patients feel comfortable returning to work and travel.
Weeks 2–4 — Looking “normal” again
- Scalp settles; redness mostly gone (can linger longer on fair skin).
- Transplanted hairs are in place but about to shed.
Weeks 3–8 — Shock loss (the shedding phase)
- Transplanted hairs fall out — this is normal and expected (“shock loss”). The follicle stays alive beneath the skin.
- Some existing native hair around the grafts may shed temporarily too, then regrow.
- Don’t panic — this is the phase that worries people most, and it’s part of the process.
Months 3–4 — New growth begins
- Fine new hairs start to emerge, often thin and light at first.
Months 6–9 — Density develops
- Hair thickens and coverage becomes clearly visible.
- This is when most people start feeling the result is “theirs”.
Months 12–18 — Final result
- Full thickness, texture and maturity. Crown areas can take the longest.
- What you see now is your lasting, permanent result.

Aftercare: do’s and don’ts
✅ Do
- Sleep elevated for the first few nights.
- Wash gently, exactly as your clinic instructs.
- Take prescribed medication as directed.
- Keep the scalp out of direct sun (wear a loose hat only when advised it’s safe).
- Stay hydrated, eat well, rest.
❌ Don’t
- Touch, scratch or pick crusts.
- Sweat heavily / do strenuous exercise for ~2 weeks.
- Swim (pool or sea) for ~2–4 weeks.
- Drink alcohol or smoke in the early days (both impair healing).
- Wear tight hats/helmets too early.
- Expose the scalp to strong sun.
Timeframes are typical — your clinician’s specific instructions always take priority.
Washing your scalp (typical guidance)
- Around day 2–3, apply the recommended lotion/foam to soften crusts (as instructed).
- Rinse with lukewarm water and low pressure — no direct strong jet.
- Pat dry gently with a clean towel or air-dry; never rub.
- Continue daily until crusts have cleared (usually ~10–14 days).
Your clinic gives you the exact product and schedule — follow it precisely.
When to contact your clinic
Most healing is uneventful, but contact your team promptly if you notice:
- Spreading redness, heat, pus or a bad smell (possible infection),
- Fever,
- Heavy or persistent bleeding,
- Severe or worsening pain not controlled by advised medication. These are uncommon, but a good clinic gives you a direct line (WhatsApp) so you’re never left guessing.
Still not sure what applies to you?
Reading about it only gets you so far. Send a few photos on WhatsApp and a physician will tell you what is actually realistic in your case.
Frequently asked questions
How long does hair transplant recovery take?
The scalp heals in about 2 weeks; full hair growth takes 12–18 months.
When can I go back to work?
Many patients return within 10–14 days, once crusts have cleared — sooner for desk work if discreet.
Why is my transplanted hair falling out?
This is “shock loss” between weeks 3–8 — normal and expected. The follicle survives and regrows.
When will my hair look normal?
Redness and crusts clear within ~2 weeks; noticeable density appears at 6–9 months; final results at 12–18 months.
When can I exercise or swim again?
Avoid strenuous exercise for about 2 weeks and swimming for about 2–4 weeks, following your clinician’s advice.
When can I wear a hat?
Only a loose hat once your clinic confirms it’s safe (often after the first several days) — never tight headwear early on.
Is the shedding phase a sign of failure?
No — shedding is part of the normal cycle. Judge results at 12 months, not during shedding.
Is it normal to still be shedding at 2 months?
Yes — shock loss runs roughly weeks 3–8, so shedding around month two is expected. New growth follows from month three; judge the result at twelve months, not during shedding.
Are the before-and-after photos online real?
Not always — it is the most-faked thing in this field. See how to read a before-and-after honestly.
