Gynecomastia — enlarged male breast tissue — is common and, for many men, a lasting source of self-consciousness that diet and exercise cannot fix. Surgery removes the excess tissue and fat to restore a flatter, firmer, more masculine chest, usually with small, discreet incisions.
What is gynecomastia surgery?
Gynecomastia is a mix of excess glandular tissue and fat in the male chest. Surgery combines liposuction to remove fat with direct removal of the firm glandular tissue, flattening and re-contouring the chest. Because true gland does not respond to diet or exercise, surgery is the reliable solution once other causes are excluded.
Gland, fat or pseudogynecomastia — and how severe
Not every enlarged male chest is the same, and the difference decides the treatment. A simple pinch test and examination tell true gland from fat.
| Type | What it is |
|---|---|
| True gynecomastia | Firm glandular tissue behind the nipple — does not respond to diet or exercise; needs surgical removal. |
| Pseudogynecomastia | Excess fat only, no true gland — may improve with weight loss, and responds to liposuction. |
| Mixed | Both gland and fat — the most common, treated by combining liposuction with gland removal. |
Surgeons also grade severity (commonly a grade 1–3 / Simon scale, from mild fullness to significant enlargement with skin excess). Higher grades with loose skin occasionally need skin tightening as well, planned at assessment.
Causes include normal hormonal shifts at puberty, ageing, weight, some medications, and anabolic steroid use — which your surgeon reviews, since a treatable cause is sometimes addressed first.
Are you a good candidate?
- Persistent enlarged male breast tissue that has not resolved with time or weight loss.
- A stable weight and good general health.
- Realistic expectations.
Your surgeon will check that there is no underlying medical cause that should be addressed first. In most men it is simply a benign, treatable condition.

How it is performed
Under general anaesthesia or sedation at Acıbadem in Istanbul, liposuction removes excess fat through tiny incisions, and firm glandular tissue is removed directly, usually through a small incision at the edge of the areola where it heals discreetly. The chest is re-contoured to a natural, masculine shape.
Recovery
A compression garment is worn to support healing and contour. Soreness and swelling settle over one to two weeks; most men return to desk work within about a week and avoid strenuous activity for several weeks. The final result appears as swelling resolves.
Risks and safety, honestly
Gynecomastia surgery is a well-established, generally safe day-case operation in qualified hands — but honesty about risk matters. Alongside the usual surgical risks (bleeding, infection, poor healing) the specific ones are contour irregularity or a slight dip if too much tissue is removed, temporary numbness around the nipple, small differences between sides, and fluid collection. Removing the right amount — flat but not hollow — is a matter of surgical judgement. At Clinic Elia the procedure is performed by Op. Dr. Ali Çetinkaya at Acıbadem, after ruling out any medical cause that should be treated first.
Results
Removed glandular tissue does not return, so results are long-lasting, provided a stable weight is maintained. For many men the effect on confidence is significant.

Frequently asked questions
Will the gynecomastia come back?
The removed glandular tissue does not return. Significant weight gain or certain medications can affect the chest, so a stable weight helps keep the result.
Can exercise fix gynecomastia instead?
Exercise reduces fat but not true glandular tissue, which is why surgery is the reliable solution once other causes are excluded.
Where are the scars?
Usually a small, discreet incision at the edge of the areola, plus tiny liposuction incisions, which fade over time.
Is it painful?
Discomfort is usually mild to moderate and well controlled. A compression garment supports healing.
How long is recovery?
Most men return to desk work within about a week, avoiding strenuous activity for several weeks.
Is it discreet?
Yes — small incisions and a private, surgeon-led process. Many men find it life-changing for their confidence.
How long do I stay in Turkey?
Usually around 5–7 days, confirmed in your plan.
Is it gland or just fat (pseudogynecomastia)?
A firm disc behind the nipple that resists diet and exercise is true gland and needs surgery; soft, even fullness is more likely fat. Examination and a pinch test tell them apart.
Can gynecomastia be fixed without surgery?
Fat-only (pseudo) can improve with weight loss. True glandular tissue does not respond to diet, exercise or fat-burners. Medicines such as tamoxifen are sometimes tried early or for a hormonal cause, but they do not remove established gland — surgery is the reliable solution.
Do steroids cause gynecomastia?
Anabolic steroid use is a well-known trigger. If you are still using, your surgeon will discuss timing, because continued use can bring it back.
What are the grades of gynecomastia?
Severity is commonly graded 1–3, from mild fullness to marked enlargement with loose skin. Higher grades occasionally need skin tightening as well.
Will there be a visible scar?
Usually just a small, discreet incision at the lower edge of the areola, which fades over time, plus tiny liposuction access points.
